with residence at Brgy. Solido, Nabas, Aklan;

That the minor child Ma. Jersey Alexa Nicole Otero is under the parental
care and support of said solo parent NENIA JEAN ARSENIO OTERO;

That I am executing this affidavit to attest to the foregoing facts for all
legal purposes and in relation to the requirements for the availment of the
programs and benefits of the Department of Social Welfare and Development
under Republic Act No. 11861 otherwise known as the Expanded Solo Parents
Welfare Act.

I, NENIA JEAN ARSENIO OTERO, of legal age, Filipino citizen, single,

and resident of Brgy. Solido, Nabas, Aklan, after having been duly sworn to in
accordance with law, do hereby depose and say:

That I am the mother and a solo parent of minor MA. JERSEY ALEXA
NICOLE OTERO, thirteen (13) years old;

That I am the only one who rears and raises said MA. JERSEY ALEXA
NICOLE OTERO who is under my sole parental care and support;

That I am not cohabiting with a partner nor receiving any support

from the father of the minor child since the latter’s birth;

That I am executing this affidavit to attest to the foregoing facts for all
legal purposes and in relation to the requirements for the availment of the
programs and benefits of the Department of Social Welfare and Development
under Republic Act No. 11861 otherwise known as the Expanded Solo Parents
