Priority care for the following:

Cognitive Disorders
1. Dementia- as a nurse, the priority nursing care for people with dementia is to provide
treatment, care and support for people with mental health problems and dementia. They
might assess you at home, and they advise you and your careers on ways of improving
your health and quality of life.

2. Delirium- The nursing priority care in delirium patients is to measure the level of anxiety
and create a suitable setting because anxiety rises in a high-stimulation environment,
ensure the client's safety, and teach relaxation activities. Maintain a cool demeanor with
the client to avoid frightening them and, more critically, to interrupt periods of denial.

Dissociative Disorders

1. Depersonalization Disorder - For clients with depersonalization disorder, the nursing


priority care is to establish a therapeutic, nonjudgmental relationship with the client,
encourage the client to recognize the depersonalization, recognize and deal with
anxiety-producing experiences, assist the patient in establishing supportive
relationships, and the patient may recover without treatment, psychotherapy, cognitive-
behavioral therapy, hypnosis, or SSRI/TCA.

2. Dissociative Amnesia/ Psychogenic Amnesia- The priority nursing care for this client is
to identify the source of stress, present reality, prescribe Benzodiazepines and SSRIs,
maintain a nonjudgmental relationship, acknowledge that memory loss is a defense
mechanism, and learn to cope with anxiety-producing circumstances.

3. Dissociative Fugue/ Psychogenic Fugue - As a nurse, it is critical to recognize stress-


related emotions, monitor indicators of hostility toward others, teach appropriate coping
techniques, utilize existing social support systems, and refer patients to psychotherapy.

4. Dissociative Identity Disorder/ Multiple Personality Disorder –There are five things that
people with this disease should not do. First, don't encourage them to create new
personalities; second, don't suggest that patients give their subpersonalities names;
third, don't encourage subpersonalities to function independently or allow the patient to
ignore certain personalities; and finally, don't exclude unlikable subpersonalities from
therapy.
Sleep Disorders
The nursing primary care for sleeping disorders is to provide meals at regular
times that correspond to the client's past patterns, in order to reinforce circadian
rhythms, which tend to deteriorate with age. Because light exposure is relayed through
the retina to the superchiasmatic nucleus, which helps to set the circadian clock, we
should also provide active meaningful activities during the day.
