Please be informed of the following enrolment process:

Incoming/New Students and Returning Students:

1. All Incoming students must have received their letter of acceptance or Notice
of Acceptance from their respective Deans prior to enrolment. Please make
sure that the LOA/NOA must include your DMSF official email address.
2. The following are the documents needed for enrolment:

3. Together with the LOA/NOA and the complete set of documents, submit them all at
the Registrar’s Office. Upon complete submission of all documents, a student profile
form and the Guidance questionnaire will be given and has to be filled up and
submitted together with the 2 x 2 ID picture in blue background. . (with collar and
with the prescribed haircut).
4. Upon submission of the filled up student profile, the enrolment form will be released
by the Registrar Staff for the signature of the following offices:

 Guidance Office - filled up questionnaire is needed for securing the signature


 Office of the Student Affairs (OSA)
 Clinic
 Cashier
 Dean
 Registrar
5. Since all students have to undergo Physical Examination by the School Physician, a
laboratory test at the Davao Medical School Foundation Hospital is required.
6. A prior appointment at the DMSF laboratory is needed. Kindly claim your laboratory
request from the School Clinic as a basis for securing a prior appointment. A
maximum of Seventy-five (75) students in the morning and 75 students in the
afternoon will be accommodated. Those who will be absent in time of their schedule
have to pre-register again. The receipt with a notation “EXTRACTED” by the
Phlebotomist assigned will have to be submitted to the School Clinic to secure their
signature on the enrolment form. You will be notified later of your actual physical
examination schedule.
7. The process of getting an appointment at the DMSF Hospital Laboratory can be done
once a Letter of Acceptance or Notice of Acceptance has been received.
8. After securing the entire signature as listed in the enrolment form, submit the form to
the Registrar’s Office.
9. The enrolment Registration Certificate (ERC) will be emailed to the student after.
Kindly show your ERC to the Professor on the first day of every subject.

OLD/Existing Students:

1. A Pre-registration Note (PRN) for undergraduate courses and the promotional report
from the College of Medicine are the basis for enrolment.
2. Students from Biology, Dentistry, Midwifery, and Nursing have to secure a PRN first
before proceeding to the Registrar’s Office for enrolment.
3. A student profile form and the Guidance questionnaire have to be filled up and
submitted together with the 2 x 2 ID picture in blue background. . (with collar and
with the prescribed haircut) All these forms are available at the Registrar’s Office.
4. Upon submission of the filled up student profile, the enrolment form will be released
by the Registrar Staff for the signature of the following offices:
