Thank you ms laca that was an interesting topic about the special provision, so after the

rule 1051 and 1052 been tackled. Now let’s turn our attention to rule 1053 which is the
report requirements. Under that, we have rule 1053.01 and here, it’s stated that
1. All work accidents or occupational illnesses in places of employment, resulting in
disabling condition or dangerous occurrence, shall be reported by the employer to
the Regional Labor Office.
Meaning, if si Dora ay nagtatrabaho sa office then not knowing biglang sumabag yung
oven nila don then nasabugan siya at nagkaron ng injury sap aa na nagcause na di
makalakad, kailangan mareport ito sa RLO, if hindi naman siya nasugatan or wala
naming reportable injury, pero still there’s a potential to cause significant harm.
Kailangan pa din ito mareport sa RLO.
Papasok ang form DOLE/BWC/HSD -IP-6 UNDER RULE1050, since ito work related
accidents, ang isusubmit na form/report under DOLE/BWC/HSD -IP-6 , ay
EMPLOYER’S WORK ACCIDENT ILLNESSES REPORT, ditto, ito yung work related
accidents/illnesses na nagresulta ng (minor injury or even illnesses).
Sino-sino ang magsusubmit? Yung under ng top management. Sino-sino ang nasa top
management? Itoy ung mga senior level executives, yung mga Board of Directors,
Managing Director, Chairman, CEO ( Chief Executive Officer), Vice-President,
President, General Manager, and other Senior Executives.
Ito ay isusubmit sa DOLE-RO on/before 20th day of the month following the date of
occurrence of the accident. Ibig sabihin On or before 20 th day of the following month.
Then yung investigation form naman isusubmit on or before the 30th day of the same
month.
San isusubmit ang WAIR? Sa DOLE regional office, ito ay isusubmit ng top
management Sino naman ang mag-susumit ng investigation report? Ang RO sa DOLE.
Ilang copies parehas? Dapat may 3 copies kang dala, dahil yung isa para sa DOLE,
yung isa para BWC at yung isa ay yung receiving copy.
At kapag hindi pa makabalik ang injured employee within the closing date of reporting,
or yung leave days, magkakaron ng estimation sa probable days, kung ilang days na
hindi pa ito makakasok. And saka lang ma-icocorect yung estimation if returned na
yung employee. Pero kung nagging accurate naman yung ginawang estimate, edi no
need to correct.

2. Halimbawa:
What if si dora ay nagkaroon ng permament total disability or binawian ng buhay?
inonotify yung DOLE-RO within 24 hours, yon ay kapag ang workplace ay naka-
encounter ng serious physical harm sa isang tao or fatality. Take note: Notify pa
lang, wala pang report na isusubmit. Pwedeng email, phone call or even by text,
basta dapat alam na ng DOLE within 24 hours.
3. After manotify at mareceived ng DOLE ang initial report about the fatalities or accidents,
saka magkakaron ng investigation ang RO within 48 hours. And yung copy ng form
DOLE/ BWC/OHSD-IP-6, dapat may duplicate ka, then aside from the duplicate dapat
meron kang isang copy para sa sa receiving.

NEXT, RULE 1053.02


1. Any dangerous occurrence, may or may not cause serious bodily harm or
seriously damage the premises of employment, shall be investigated and
reported ng employer or top management sa DOLE RO/ RLO thru FORM
DOLE/BWC/HSD-IP-6. As long as there’s a potential harm in the area, it’s
necessary to take action, we shoul be proactivenot reactive.
Ano ano ba yung dangerous occurrence na need mainvestigate and report:

a. yung catasthrophic failure ng boiler, either cause ng overheating or yung excessive
pressure, kaya nagkakaron ng explosion.
b. Air compression tank dahil sa mtaas na temperature at pressure.
c. Bursting of a revolving wheel, or yung grinding wheel kapag may clogged or
damage, it causes severe hazard na or yung explosion sometimes causes
operator fatality.
d. Pag collapse ng crane (yung sasakyan na ginagamit paglift ng loads) or derrick (
mast extending base lang, tanging boom lang yung movable, stable lang, parang
poste lang ganon) dahil sa excessive loads di na nakayanan yung bigat, or yung
overturning na nawawala na sa balance.
e. Explosionor fire causing damage. Or it also called conflagration, extensive fire na
to ay Malaki na ang nadedestroy either whole property or community.
f. Electrical short circuit , yung explosion ng machineries, other scenario na
nagcacause ng structural damage. Of course kapag ganyang damage na,
talagang magkakaron ng stoppage and misuses sa isang area na may
dangerous occurrence for not less than 24 hrs. Para macheck,investigate at
mareport ito sa RLO.

Paalala lang na reqired magsubmit ng report sa DOLE kapag nakaencounter ng work


related accidents or illnesses, dahil according to DEPARTMENT ORDER NO. 198
SERIES OF 2018 . Implementing the rules and regulations of Republic Act No. 111058
entitled An Act Strengthening Compliance with Occupational Safety and Health (OSH)
Standards and Providing Penalties for Violations Thereof.
Kapag hindi nagsusubmit ng mga reportorial requirements sa DOLE, pwedeng ma
penalize ng 30,0000 pesos per day until compliance. And if ever naman now mo lang
nalaman na required pala ang pagsasubmit ng work related accident report, eh diba
may mga due date tayong namention kanina, no worries pwede ka pa din magsubmit,
basta next time magpoprovide ka na ng report and no excuses na.

So, after the rule 1053 report requirements, we should help employers recognize
workplace hazards and correct hazardous conditions. And this will be dicussed by ms
Manalo, under rule 1054, the keeping of records.

CONCLUSION
1. When the report is not admissible, special provisions may be used during times
to serve as an evidence.
2. Under reporting of work-related accidents and illnesses. 
3. There are also report requirements that are strictly should be followed in which
they will be stored and made secure.
4. During a work-related accident or illnesses, it should be recorded according to its
proper classification. Proper recording helps with proper filing of employees records for
future use.
5. There are even computations on how a person should be rated during illnesses
or injuries. Provides information on accident causes which contributes to high
injury rates.
