RELEASE  AND  WAIVER  

 
I  hereby  authorize  Malayan  High  School  of  Science  and  its  PR/Marketing  partners  to  photograph  
and  interview  me,  to  record  a  video  or  audio  of  me,  and  to  use  the  narrative  I  provided  related  to  
Malayan  High  School  of  Science  for  purposes  of  publicity  and  promotion  of  Malayan  High  School  of  
Science  and  its  programs  and  services.  I  unequivocally  consent  to  the  use  of  my  likeness,  image,  and  
other  materials  (in  whole  or  part)  in  any  and  all  publications,  advertising,  news,  blogs,  and  social  
media  channels  that  Malayan  High  School  of  Science  and  its  partners  use  to  transmit,  publish,  and  
highlight  related  materials.  I  hereby  agree  to  provide  Malayan  High  School  of  Science  with  a  
worldwide  royalty-­‐free,  non-­‐exclusive  license  to  use  in  connection  with  the  permitted  purposes  any  
and  all  materials  I  provide,  including  digital  images,  quotes,  and  interview  details  provided  
hereunder.  I  agree  that  neither  Malayan  High  School  of  Science  nor  its  partners  owe  me  any  
compensation  for  the  materials  and  information  that  I  have  consented  to  providing  in  this  
agreement  or  their  use  in  connection  with  the  permitted  purposes.  I  further  understand  and  agree  
that  these  materials  may  be  kept  on  file  and  used  by  Malayan  High  School  of  Science  and  its  
partners  for  potential  future  permitted  purposes  and  further  agree  to  release  Malayan  High  School  
of  Science,  its  directors,  officers,  employees,  and  partners  from  any  and  all  liability  arising  from  or  
in  connection  with  the  taking,  use,  publication,  or  dissemination  of  such  materials.
