SUBJECT : NOTICE TO EXPLAIN

This refers to report that have reached the management concerning you and one
other employee who has been allegedly involved in “Padding, kitting, lapping, and
tampering or non-issuance of official receipts, invoices and bill; procuring
fictitious receipts to support reimbursement or liquidations, payment to
fictitious and the like and Not remitting cash receipts or of any cash items”
freffering to the following information:
. As you may be aware, these acts constitute sufficient ground for disciplinary
action, including suspension and termination from service.

You may explain within 48 hours upon receipt of this notice on why you should
not be penalized for allegedly violating the Hospital Code of Discipline, Act. 15 & 16 -
“Violation of Official Receipts Policies & Non-Remittance”.

Please be reminded that failure on your part to comply on the prescribed period
shall construe as a waiver of your right to be heard and we shall act on the matter
based on the records we have on hand.
