Dear Sir / Madam:

Please be advised of my desire to alter/renovate and/or my unit in accordance with and


pursuant to the Master Deed and Renovation Guidelines. In support of my application, I
herewith submit the following documents:

(a) Preliminary lay-out plan, ceiling plan, electrical-mechanical plan, etc.;


(b) Perspective plan showing general design, finishes and specifications;
(c) Lists of machineries, tools and equipment to be used.

Should this application be approved, I agree to follow and abide with all the terms,
conditions, rules and regulations of the Building and the Association.

2. Alteration/renovation works will start on ____________________ and will be


completed on _________________________ by my General Contractor,

______________________________________ and shall be in accordance with the
Renovation Guidelines.

3. I will impose upon my contractors/subcontractors to abide with all the rules,


regulations, instructions that may be given/issued by the Building Manager. I will
dispose of all rubbish and debris (thru my contractor/subcontractor) at my own
expense and account, every day.

4. All the workers will be registered with the Building Administration Office by the Unit
Owner/Contractor and shall be issued with proper ID card.

5. Access to the unit for workers and materials shall be thru the service elevator and/or
service stairway only. In no instance shall the passenger elevator be used in hauling
and transporting of workers, materials and garbage or debris.
6. In case there is no power yet in my unit, I shall request for temporary connection of
electrical facility for basic lighting and for minor tools and equipment. The cost of
electricity will be for my own account and expense.

7. All electricity and water consumption consumed by my Unit, during the renovation will
be for my account.

8. No overtime works is allowed without the prior approval of the Building Manager.
Request for overtime must be filed at least 24 hours before the intended date of
overtime work.

9. Upon approval of this application, I shall deposit a certified check in the amount of
Fifty Thousand pesos (Php 50,000.00) payable to the Condominium Corporation or
to the Developer to cover for whatever damages that may be incurred and such other
expenses that may be incurred in connection with the renovation/alteration as well as
other charges to wit:

 Number of working days used for the 1 st month of any renovation, a fee of Php
50.00 per day will be charged.

 Number of working days used for the 2 nd month and 3rd month, a fee of Php 100.00
per day will be charged.

 Number of working days used for the 4 th to the 6th month, the fees shall be
increased to Php 150.00 per day. Lastly, if the renovation reaches seven (7)
months and onwards, the fee shall be Php 300.00 per day of work.

10. I agree to pay an amount of Two Thousand Five Hundred Pesos (Php 2,500.00) as
payment for the Renovation Permit Fee for any major renovation as stipulated on the
guidelines.

After deduction of whatever damages or other fees incurred, the balance of the
renovation bond will be refunded to the unit owner without interest.
