Survey Questionnaire on Blended Learning and Academic Performance of

Humanities and Social Sciences (HUMSS) Learners

Statement of the Problem

The purpose of this study is to determine the impact of blended learning on the

academic performance of Humanities and Social Sciences (HUMSS) learners at Dr.

Juan A. Pastor Memorial National High School.

Specifically, this sought answers to the following questions:

1. What is the profile of the respondents in terms of:

1.1 sex;

1.2 grade level; and

1.3 general average on the last school year?

2. What are the impacts of blended learning on the academic performance of the

respondent in terms of:

2.1 written works;

2.2 performance tasks; and

2.3 summative test scores?

3. Is there any significant relationship between the respondent’s assessment of the

impacts and the profile variables?

4. What recommendations may be given based on the findings?
