1.2. Introduction to MacroPATH and MacroPATH Basic

MacroPATH and MacroPATH Basic were designed to obtain images of histological samples and to ensure
their complete, safe and reliable handling.
MacroPATH and MacroPATH Basic differ by the fact that MacroPATH Basic does not include the PC Monitor
(see chapter 2.1).
The system has been designed for use in a laboratory and can therefore withstand the harshest laboratory
conditions (indoor use only).

MacroPATH's measurement function is strictly linked to the calibration performed by the

operator and is not controlled by the unit's software. That is why the measurements of
the sample obtained with MacroPATH CANNOT be taken into account for diagnostic
purposes. These values are purely indicative.
Indeed, a ruler must be placed near the sample (see image below) or a gauge must be used to obtain
a diagnostic value for the growth of the tumour, the extent of the lesion, etc.

The system must be calibrated in order to perform measurements with MacroPATH (refer to chapter 3.6.6
for the calibration procedure).

3.4.4. LIVE area Function

In LIVE it is possible to create/retrieve a case, acquire images, take a video or audio and edit supplementary
camera settings.

When the camera has been on for a long time, the buffer might automatically reset, during this procedure the
camera switches off and back on automatically as explained below.

Currently, the “camera wake up” procedure is: the live screen is stopped and the following message is
displayed. The maximum time is 90 seconds, during this period do not press any button, just wait.
This procedure may happen during the live stage or during editing.

Wait for the end of buffering, after which time you can use the camera again.

To extend the life of the camera, it goes into standby by default after one hour (see “Settings” menu, chapter
3.6.11). When MacroPATH remains idle for 1 hour, the camera therefore switches off and the live screen
turns black. If you want to switch on the camera, press its icon, as shown below.

When the camera is on “Buffering” may appear for a few seconds.

If you want to disable the camera standby, please contact our service department at
customersupport@milestonemedsrl.com.

3.4.8. Basic camera settings

Press and hold the camera icon for at least 3 seconds to open the camera settings window. In this window it
is possible to set:

3.4.8.1 Advanced Camera Setting

• SNAPSHOT QUALITY: to change the snapshot quality.

• OPERATING MODE: shooting method setting
✓ Automatic Shooting Mode: the camera chooses the basic settings automatically
✓ Semi-Automatic Shooting Mode: semi-manual entry of camera parameters
F Number: focal ratio setting, we recommend setting an average value to achieve an optimal
light/field depth ratio (F 6.3 is recommended).
ISO: image grain setting, we recommend setting a low value to achieve greater image detail
(ISO80).
Exposure Compensation: manually changing the brightness of the image, set at a value
other than 0 if you want a darker or lighter image (0 is recommended).
White balance: white balance setting (light source selection)
During installation it is NOT necessary to perform white balance setting for the camera. The
digital camera will auto-balance the white according to the light conditions where the
MacroPATH is positioned.
The user may select “AUTO” where the camera automatically adjusts the white balance or it
is possible to select the type of laboratory light source from the options in the list shown
below, so as to obtain the most realistic colour.

40
MacroPATH and MacroPATH Basic QX Series MM161-001 – Operator Manual

“Auto” is selected by default and it is indicated when you are not certain of the type of light
source of the cutting station.
These changes are applied after pressing “Save and Close” and it is not necessary to reboot
the software.
If there are additional cameras, modify the settings for each cam separately.
The changes are saved in the configuration in use (see chapter 3.6.1).
• HDR Shooting Mode: if necessary, the camera takes various shots and overlaps them
• Firmware Version: this is the camera’s Firmware version

This chapter explains how to set the capture quality (5-10-20 Mpx) but the image size also depends on the
compression quality (to set the compression quality see chapter 3.6.4).
Milestone suggests setting a snapshot quality of “10Mpx” and “Standard” compression.
This is a good compromise between file size (disk space) and image quality.
For images used for publications (articles, documents, etc.) Milestone suggests setting a capture quality of
“20Mpx” and “Full” compression.
The table below indicates the image file size in the various configurations:

Compression quality
Standard Full Low
(medium) (high) (low)
Snapshot quality
20 Mpx 2.3MB 12.7MB 1.8MB
10 Mpx 1.5MB 7.4MB 970kB
5 Mpx 750kB 4MB 520kB

Note that the space required for saving each image is twice that shown in the table above. In fact, the
system automatically saves each image in both JPEG and m5p formats.

changes. See chapter 3.6.6 for the calibration procedures. MacroPATH's measurement
function is strictly linked to the calibration performed by the operator and is not controlled
by the unit's software. That is why the measurements of the sample obtained with
MacroPATH CANNOT be taken into account for diagnostic purposes. These values are
purely indicative.
Indeed, a ruler must be placed near the sample (see image below) or a gauge must be used to obtain
a diagnostic value for the growth of the tumour, the extent of the lesion, etc.

If there is no calibration, the <?> symbol appears on the image.

3.6.4. Data storage settings

Click on the Storage button to either change or add new storage folders (up to 3). The system automatically
saves files in each folder selected.

For appropriate installation, specifically with multiple stations (when several MacroPATH units are installed or
installed in the office PCs with the “Editing” license), it is advisable to have the unit save data on a shared
folder (a server, for example). For correct system configuration, the first path needs to be the shared folder
located on the server. This folder must be set up for full control for each account (read/write/edit) and for
each PC running the MacroPATH software.
Accordingly, the second path should be a local folder (located on the PC’s internal hard disk).
For Windows 7 and Windows 10, a default folder (the user can obviously change it):
C:\Users\Public\Documents\Milestone srl\MacroPath5\IMAGES

As shown in the picture below (which illustrates the example of the first path):

\\SERVER\Pathological Anatomy\Laboratory\MacroPath\IMAGES

The local folder will ONLY store the files created and opened by the MacroPATH you are using. The shared
folder on the server will collect ALL the files created and managed by all the workstations/units.

Milestone strongly recommends storing the files on a server with an automated backup
procedure. Contact your local Network/IT Manager.

Data compression: you can define three different image compression levels: Full Quality, Standard and
Low file size.
See chapter 3.4.8 to select the best degree of compression depending on image quality.

53
MacroPATH and MacroPATH Basic QX Series MM161-001 – Operator Manual

High definition this function is not necessary for the camera (it is developed for previous camera models).

Enabled shared folder management

Shared folder management is useful when several MacroPATH stations are working on the same files. In
fact, with this function enabled, when a case is opened by a station and another MacroPATH station tries to
open it, an error message is displayed, informing that the case is already opened by another System and
displaying its IP address and when the case has been opened. An example is shown below.

Before using this function, please contact your Technical Department and
customersupport@milestonemedsrl.com.

3.6.5. Camera Settings

Click the “Camera” button to enter the camera settings page

Select the required camera (“Cam1” or “Cam2”, “Cam3”, “Cam4” and “Cam5”
depending on how many there are).

Click on Camera short name to edit the camera name.

Select:
• Trust external calibration: MacroPATH to use the calibration factors of previous files and not those
of the software.
• Force calibration: MacroPATH will require calibration to be carried out on every new image (helpful
when changing capture camera height).
• Auto measure: function not available.
• Vertical Flip Image: the image is flipped along the vertical axis.
• Horizontal Flip: the image is flipped along the horizontal axis.
• Priority: If two or more cameras are connected to MacroPATH, you must choose the selection
sequence.
It is essential to set the priority when working with multiple cameras. ‘Live’ shows the

• Foot Pedal Control: enable the foot pedal (if connected) by choosing the serial number from the list.
• Keypad: not enabled.
• Calibration Procedure: by pressing this button it is possible to calibrate the unit, as explained in the
following chapter.

3.6.6. Calibration
Click the “Calibration Procedure” button and position the ruler on the work surface.
We recommend placing the ruler starting from the “0 mm” measurement on the left-hand side; also, the ruler
should be placed HORIZONTALLY at the CENTRE of the live image.
Press Focus at the top right if the image is not focused, then press Go to take the first picture required for
calibration.

Insert the distance (in mm) between the two edges of the image (see picture). Then press NEXT.

Repeat the action 7 times (i.e. for the 7 zoom levels)

At the end press “Save”. The calibration procedure is now complete.

The calibration procedure is required every time the camera position or lens type are
changed.
Every camera has its own calibration; therefore, each camera needs to be calibrated
separately from any other that may be connected.

3.6.7. Third-party settings

It is possible to create shortcut buttons to launch external software (up to 3 different external applications,
not included):

Select the “External application” checkbox, as shown below. The image shows launching the “Notepad”
application as an example. To do this, fill the fields under “External application1”:
- “Application”: this is the address, in this case “C:\Windows\System32\notepad.exe”
- “Process”: this is the name of the application, in this case “Notepad”
- “Image”: it is the icon that will be displayed when the application is opened, it is required to select a “png”
image
- Label: in this case “Notepad” (which appears on the icon created).

Shutdown MacroPATH after launch: if selected, when the external application opens, MacroPATH closes.

Disable MacroPATH after launch: if selected, when the external application opens, MacroPATH is
disabled.

When the “Disable MacroPATH after launch” box is selected, a specific program (not
included, to be set by the user) must be installed to let the user go back to MacroPATH.
Otherwise, going back to MacroPATH is not possible and the user will need to restart the
MacroPATH application by means of Windows’ red “CLOSE” cross.
DO NOT select when in doubt

When all the fields described above have been filled in press “Save”. The icon referring to the application is
displayed under “Files”. In the example below, a “Notepad” icon appears.

56

If you press the button, Notepad opens automatically.

It is also possible to customise the “Exit” button in the “Customize exit button for already running processes”
section.
Under “Process” add the name of the program to be highlighted, import the .png image to “Image” that will
replace the button image, Exit and under “Label” enter the desired name for the new button.

When the "Exit" button is pressed, the program will be highlighted (only in cases where it has been
minimised on the status bar).

If the following checkbox is enabled:

when the exit button at the top of the screen is pressed, MacroPATH is minimised but not closed.
However, if the button is held down for at least 3 seconds, MacroPATH is closed.
You can enable this checkbox when MacroPATH is opened directly by its button (not by an external program
or an application) or also when MacroPATH is started from the command line.
However, you cannot enable this checkbox when working as a Twain device. In this case if you wish to
minimise MacroPATH upon exit, you are prompted to select the “Volatile” mode in “Settings→Third party” in

address, only the images with the saving date and time will be.

The following are some specifications regarding the TWAIN connection:

1. The MacroView DM TWAIN connection does not support Citrix technology of the desktop display nor
any other virtual machine. It was not developed or tested for these environments.
2. The window of the MacroView DM application always opens maximised. It can be re-sized, but it does
not retain the change between two successive Twain sessions.
3. If the operator closes the MacroView DM window during the Twain session by pressing “X” on the status
bar, the window closes immediately and the open Twain session is no longer managed. All the images
that must be transferred might be lost.

Also, in “Settings - Third party” select Enable Twain acquisition to import images from an external imaging
device (such as a microscope, not included). The device must be Twain to be able to send the images to
MacroPATH.

If enabled, a new “From Twain” button appears on the Live screen.

A MacroPATH case must be open to use the “Acquisition from Twain” button. An image will be imported in
the case. Measurements by using the calibration of the MacroPATH camera will not be possible on this
image.

To activate any setting performed in the "Settings-Third party" page, press Save in the lower right-hand
corner of the Settings page.

4. MacroPATH as MULTISTATION VIEWER

4.1. How to install MacroPATH as viewer

It is possible to install MacroPATH in all the desired computers without license, using the software provided
in electronic format (for example, the office of every individual pathologist).
In this case MacroPATH will only work as a viewer, simply allowing images to be retrieved, by using either
the barcode or manually entering the case name. The images can only be viewed but not edited.

To carry out any edits on the images (such as adding notes, sections etc.), a MacroPATH software license
must be available. Contact your local dealer.
MacroPATH as viewer is the same software installed on the MacroPATH unit and it does not need a new
license. Therefore, the user is authorised to install MacroPATH software as Viewer on any PC.

For license activation see chapter 3.6.3.

MacroPATH Viewer can be installed on any computer: using the MacroPATH USB key and open the
MacroPATH folder. Click on Set Up installer and follow the instructions set out in chapter 2.7 to proceed with
MacroPATH software installation.

The first time MacroPATH is installed on a PC the FULL feature license is provided for a
trial period of 90 days free of charge. After 90 days, if a license is not activated you will
only have access as MacroPATH Viewer. Refer to chapter 6.20 for further information on
licenses.

65
MacroPATH and MacroPATH Basic QX Series MM161-001 – Operator Manual

4.2. PC settings for MacroPATH Viewer

Milestone strongly recommends that the following procedures be carried out by your local
Network/IT Manager.
Before proceeding with the following instructions, it is required to connect the PC with the
licensed MacroPATH software installed and the PC with MacroPATH Viewer to the local
network, setting it with appropriate parameters (see chapter 3.6.4)

MacroPATH Viewer can access files acquired by the laboratory-installed MacroPATH only if both have the
same saving folder set in Setting storage, as per these instructions:

1. Launch MacroPATH viewer

2. Click the “Settings” icon on the screen
3. In the Storage window click on the first Browse button as shown:

\\SERVER\Pathological Anatomy\Laboratory\MacroPath\IMAGES

4. Set the same address contained in Setting storage of the laboratory’s MacroPATH.

For correct installation it is essential to enter the server folder shared by the MacroPATHs
in the first address, as shown in the figure

It is now possible to work from two (or more) work stations. When a new case is added from the histological
laboratory, every additional work station is able to view and print the same images.

Milestone strongly recommends storing the files on a server with an automated backup
procedure. Contact your local Network/IT Manager.

4.3. Integrating MacroPATH with LIS systems

The MacroPATH software offers perfect integration with the LIS systems. Different methods can be used to
perform the integration:

➢ TWAIN protocol

To use the TWAIN protocol, refer to chapter 3.6.7.

➢ Command row

With the simple command string, the LIS system is able to start MacroPATH to support the “workflow”.
The command that LIS can send to MacroPATH consists of two parts: the connection to the MacroPATH
software and the patient identification code (ID).

For example:
from the default installation directory
MacroPATH software can run:
C:\Program Files (x86)\Milestone srl\Macropath5\ Macropath
Patient ID: abc

The full command will be:

With this command the MacroPATH software is launched automatically by immediately opening patient file
“abc”.

The user can directly acquire all necessary files: photos, audio, video, which will be saved in the file storage
folder (in this example “abc”) and kept in the selected saving directory (see “Storage” in the “Setting” menu).

When these operations are completed, the patient data will be available for the LIS system.

Important note for Command Row

To import data into the LIS system there needs to be an additional application to manage
the patient folder based on customer requests.
This function is normally already enabled in the LIS configuration.

67
MacroPATH and MacroPATH Basic QX Series MM161-001 – Operator Manual

Flow and requirements for integration with command row:

Command row with

patient ID

The folder identified with

patient ID is created in
the saving directory

Data are imported by an

additional application in
the LIS system

Shared folder

For each patient, the software creates a specific folder identified with file name/number containing all of the
images, audio and video created with MacroPATH.
Additionally, the software also creates the XML file of the session in this folder, with a lot of information,
including: Patient ID, file opening and closing time.

How MacroPATH saves the images

For each specific case it is required to enter the file name/number, using the software’s interface, by
manually typing it with the keyboard or using the barcode reader. MacroPATH does not automatically
generate any identification code for the file, so to start the system the user is required to enter the specimen
identifier file name/number.

The MacroPATH system can also receive the file name/number directly from the LIMS system.

Image acquisition

Refer to chapter 3.4.6.

Where and how images are saved:

MacroPATH files are saved (by default) in the path/directory:

C:\Users\Public\Documents\Milestone srl\MacroPath5\IMAGES
By default, this directory is created in the hard-disk C, but the archive can be saved wherever you wish. We
strongly recommend placing the Images folder in a different position on the local network (refer to chapter
3.6.4).

The example provided below shows how 5 images taken from file ABC are saved in the directory
(C:\Users\Public\Documents\Milestone srl\MacroPath5\IMAGES)

All the images are saved in .JPG format. The MacroPATH software automatically saves the images with the
ID name (file name) as shown below:

68

ABC_0001.JPG
ABC _0002.JPG
ABC _0003.JPG
ABC _0004.JPG
ABC _0005.JPG

All the images are then available to be recovered and displayed by any other software.

The XML file shown below was created automatically for the ABC file used in this example:

69
