Diabetes or endocrinopathy admitted in the COVID-19 ward

Summary
The Covid-19 pandemic confronted us with unknown clinical pictures, also in diabetology and
endocrinology. Sharing clinical experiences is therefore of enormous importance. Actually, information
about the care given in the Covid-19 ward (in contrast to that provided in the Emergency Room/ICU) is
still sparse. The last weeks we built experience and gathered knowledge while giving hospital care to
patients who had a pre-existent endocrine disease (and diabetes; most patients suffered from a type two
diabetes). In our contribution we presented our insights obtained from this intensive period obtained in
the Covid-19 ward.

Insights
Covid-19 Virus and its effect towards the people’s lives has left us laborious in adapting new
normal. New normal includes wearing of face mask, 5 meters social distancing, wearing of face shields
and other protocols that helps prevent the spread of the said virus. Although it is not a bad thing, we
cannot deny the fact that it is overwhelming especially that everyday we face challenges not to adapt the
virus. However, there are really some people just find it difficult to follow these protocols given that most
people don’t believe that Covid-19 virus exist. Despite of this issue, we, who believe that Covid-19 has
killed thousands of people every single day, can only follow these rules and encourage everyone to also
follow.

This article shows concern on diabetic patients admitted to Covid-19 ward. When a person has
the virus, it is not only the virus that is fighting with the body but mostly shows some complications. This
is applicable especially to those senior patients aging 65 years old and above. Diabetes individuals are just
as likely to contract the coronavirus as people in the general community. When COVID-19-positive
individuals with pre-existing diabetes are admitted to the hospital, their clinical trajectory is frequently
more complicated, and there is a corresponding increase in morbidity and death. It is because
Theoretically, increased expression of ACE2 receptors in the lungs during hyperglycemic conditions
explains this (diabetes animal models). In vitro studies revealed that these ACE2 receptors significantly
promoted coronavirus entry. In mouse diabetes models, the degree of ACE2 receptor expression is
differentially regulated throughout various organs (for example, more expression in the kidney cortex
than the heart), with an increased expression in a glycemic state. Insulin treatment, however, did not
reduce tissue ACE2 receptor expression in the lung when used to lower high glycemic conditions (only
the circulating ACE2 protein decreased). The article would like to show to everyone that is having
complications like Covid-19 with diabetes is a very difficult challenge to surpass since it could also lead
to death. The health care staffs and providers who are treating these patients needs more courage and
intelligence on order to provide proper treatment.
