IDENTIFYING DATA AND REASON FOR REFERRAL:

Martin is a 21-year-old male who was referred for an interview to check his
current well-being. Martin is currently having a hard time with his academics.

SOURCES OF INFORMATION:

Background information was obtained from the client himself. This information
was obtained from an interview. This information appears to be reliable and valid.

BACKGROUND INFORMATION:

Current Concern:

Currently, Martin is having a hard time with his academics, and it’s causing him a
lot of stress. This affects his social life in terms of having less time spent with his family
and friends.

Medical and Development History:

Pregnancy and birth history: Martin reported that his mother was 30 and his father
was 30 at the time of his birth. There were no complications of pregnancy or delivery
reported, and he was born by cesarean section. His birth weight was normal.

Developmental history: Martin had no problems during his infant stage. However, his
appetite was reported as poor. Developmental milestones were reported as within the
normal range for language and gross motor development. Moreover, he has normal fine
motor coordination. Unfortunately, development of bowel control was somewhat
delayed. He had numerous accidents as a child, including with soccer and basketball.
These resulted in injuries.
Medical history: During his childhood, he had asthma.

Family and Social History:

Martin’s father is a contractor, and his mother is a stay-at-home mom. According


to him, he has a strong relationship with his parents. Because of their strong bond,
Martin can freely share his problems with his parents. Similarly, he also has a strong
relationship with his older brother and younger sister. He mentioned that they all share
their own problems with each other, and they also help each other overcome these said
problems. He added that his family on both sides had no history of any mental illness
that they knew of.

Summary of Previous Evaluations:

Unfortunately, Martin didn’t have any history of previous evaluations.

BEHAVIOR OBSERVATIONS

Behavior During the Interview:

Martin was an attractive young man who readily established an easy rapport and
was open and forthcoming about his struggles. He was evaluated for two hours. He
willingly answered the questions and failed to show signs of resistance, hyperactivity, or
distractibility.

Home: Martin indicated that he enjoys playing computer games and spending time with
his dogs.

School: Martin told me that he’s always on time in his class. He also mentioned that he
always gives his best in class. However, he is currently having a hard time with his
academics because he feels that he’s overwhelmed with assignments.

TESTS ADMINISTERED:

An interview was used to collect the information about the client's current
situation.

RESULTS FROM THE INTERVIEW:


Using information from the interview, Martin is currently experiencing an
above-average amount of stress from his academics. Another factor that caused his
stress was being away from home. Overall, the stress he’s currently experiencing is
common for his age, and this kind of stress usually passes over time.

SUMMARY:

Martin, a male 21-year-old, was recommended for an interview to see his current
well-being. He is currently struggling in his academics. We quickly bonded during the
interview, and he was honest and transparent about his difficulties. He answered
questions voluntarily and did not display any signs of resistance, hyperactivity, or
distractibility. Martin is currently experiencing above-average stress from his academics,
according to information from the interview. The fact that he was away from home
added to his stress. Overall, the stress he is presently feeling is typical for his age and
typically passes with time.

RECOMMENDATIONS:

Martin should make an effort to make new friends who can assist him with his
studies in order to best meet his needs. He should also get in touch with his family and
friends more frequently to help him deal with his homesickness. Whenever he
experiences homesickness, he should text or call them.
