PATIENT’S OUTCOME CRITERIA
ASSESSING
Patient is active and no signs of fatigue but will
feel pain when there is disturbance at his left
elbow. Patient is at bed, sometimes sitting on the
watcher or guardian’s chair while watching
YouTube. Patient is seen smiling while watching
videos from his phone and will seldom talk to me.
PLANNING
Plan how the patient will schedule a follow-up
appointment.
Determine the issues that demand immediate
action or an expert visit.
Indicate whether the patient needs to get the
results of any more testing.
investigations and actions that the receiving
provider should do once the patient has been
released.
After receiving instruction on how to take their
medication, the patient will feel more secure
taking it as prescribed.
The patient will be able to list activities that will

NURSING ORDER
Instructed the parents to give the prescribed home
medication to his child at the right time.
Advice the parents to assist his child to do
blowing through a straw into a glass of water.
Instruct the mother of the patient to assist his
child to do daily walking.
Advice the mother of the patient to make sure his
kid gets a Plenty of rest.
Instruct the parent to assist his child to drink lots
of liquids.
Advice the parent to assist his child to wash
hands frequently and thoroughly.
After 1 week, notify the patient, patient’s parents
or guardian to return for the scheduled follow up
examination on the specified day and time.
Advise the patient’s mother to express her
observation about the condition of the patient.
Advise the patient’s mother to tell what she
observes adverse effect of the medication.
Contact healthcare professionals
increase her independence in carrying out her
ADLs with safety and safeguards after 15 to 30
minutes of health education.

IMPLEMENTING

Advise the patient to take pain medicine exactly

as directed.

Tell the patient to take special care when

showering. Follow your healthcare provider’s
instructions closely.

Instruct the patient to not rub the incision, or

apply creams or lotions to it and to avoid from
falling while showering, sit on a shower stool.

Advise the patient to tell all of his healthcare

providers—including his dentist—that the patient
have a pin in your elbow.

Tell the patient to seek care immediately when

arms feels warm, tender, and painful or may look
swollen and red.

Advise the patient to call for emergency when

there are any negative or adverse effects that may
happen.
EVALUATING

Goal was met as patient was happy to have a

successful operation. He verbalized said “yey!
The pain will go away someday”.
