Good day!

We, the Grade 12 students of Computer System Servicing (CSS) and Dressmaking
(DM), would like to ask for your permission to allow us to conduct our survey on
December 5-7, 2022, as a requirement and performance task in our subject, English
for Academic and Professional Purposes (EAPP).
The survey will last only 10 to 15 minutes. Our respondents include teachers and
students in Senior High School. We apologize for the interruption and inconvenience
we may cause during class hours.
Rest assured that the data gathered will remain confidential and for academic
purposes only. We are hoping that this will merit your approval.
