Dear Sir/Madam,

With due regard and humble submission, I Tanulata Mohanty, aged about 68 years, W/O- Sri
B.B. Mohanty, Plot No. A/L-118, VSS Nager, P.O/P.S- Sahid Nager, Bhubaneswar- 751007,
Dist- Khurda, (Senior Citizen Redg No. 54), Odisha wanted to draw your kind attention for
necessary action in the below-mentioned matter.

Kindly treat this as a letter of Complaint regarding my treatment at Kalinga Hospital Ltd from
Dr. Gautam Panda Regn.No 10640 (Orissa) and his negligence which resulted in the loss of my
eyesight of the left eye.

BACKGROUND

On 18th June 2013 at 9 am I have visited Kalinga Hospital Ltd, Bhubaneswar for consultation on
the problem in my left eye. Dr. Gautam Panda checked my eye and advised for instant “retinal
detachment” surgery on the same day otherwise I will my loose vision as “Macula” was attached.
He estimated the cost of surgery 21,000/- and said I would be discharged within 3 to 4 hours
after the surgery. So the same was deposited by me and got admitted.

At about 7.30pm I was taken to OT which was 10hours after consultation. Then from OT, I was
taken to ICU at about 12.30am of 19.06.2013. I was there in ICU till 4 pm and then was admitted
to the hospital for two days. Thereafter I was discharged on 21.06.2013 with total expenses
65,816/-. With follow up treatment till 22.07.2013.

But I was experiencing discomfort, pain and less vision in left eye so, on 25.07.2013, I consulted
Kalinga Institute of Medical Sciences (KIMS) Bhubaneswar, there it was detected that previous
surgery was not successful and the cornea was defunct due to the wrong method adopted by the
doctor. So, I decided to re-consult the case. So, I visited Aditya Jyot Eye Hospital Pvt. Ltd.,
Mumbai on 05.08.2018, there also the same was detected and I have to undergo surgery for the
same.

COMPLAINT

I would like the following points addressed in response to this complaint:

 I was told that I would be discharged within 3 to 4 hours after surgery, but it took 2 days
to get a discharge and I was in ICU for almost 24 hours. It is a general right of the patient
and his family member to be informed about the condition. Why I was kept under ICU if
things are alright and why my condition was not informed?
 In follow up treatment after surgery I complained of pain, discomfort, and less vision,
why my complaint was not taken seriously?
 I was asked to take follow up treatment but why Dr. Gautam never informed me about
cornea defunct?
 It is the duty of a Hospital to inform the expenses prior to admission, why details were
not given when 21,000/- were deposited?

WHAT I WOULD LIKE

As a result of this complaint, I would like the authority to take disciplinary action against Dr.
Gautam Panda Regn.No 10640 (Orissa) and Kalinga Hospital Ltd, Bhubaneswar. I want the
authority to look whether proper procedure was followed before, during and after surgery. If
everything was followed then why I have to lose sight of the left eye. and I also emphasize your
august authority to take further necessary steps as deem fit and proper in the interest of justice as
it is a matter of deep concern.
