Subject: - Regarding comments on the queries made for grant of medical bill in r/o My Mother
Maruna Devi I-card No. HHS1.0701338227.

Respected Sir,

In reference to your office letter dated 29/11/2023 regarding queries made for grant of
medical bill in r/o My Mother Maruna Devi I-card No. HHS1.0701338227. The detailed comment on
the queries made point wise are as follow;

1. The original Money receipt against the final bill is attached here with as enclosure.
2. The supporting original report for electrolyte test of Rs.500/- dated on 28.10.2023 is also
being attached herewith as enclosure.
3. The Medicines were purchased from Depee Medical & Aradhya Avshadhi in emergency and
the store had not provided me GST bill for the medicine purchased. As the GST number of
Depee Medical & Aradhya Avshadhi is not available
4. In r/o the supporting original doctor advice for medicines of Rs.1285/- on dated 20.10.2023
it is to intimate after discharge from the hospital urinal problem occurred and doctor had
advised to buy medicines for the urinal problem after every 15 days. Due to which
documents for doctors’ advice after each 15 days is not available.
Therefore, it is requested to grant me the medical bill after deducting the medicine bill of
Rs.1285/- purchased from Depee Medical & Aradhya Avshadhi.
5. The claim intimation must be submitted within 48 hrs but the case was of emergency as my
mother is very old and her age is 96 years who accidently fell off the staircase and her leg
fractured. I took her to hospital in the same condition. As the case was critical, I was not
able to inform instantly about the incident to anyone but later I informed the branch about
the incident.
6. My mother was kept under the observation of the doctor even after her discharge, because
due to her old age recovery was quite slow. As a result, proper documents regarding
Medical Bill could not be provided by Hospital in time and led to delay in submission of
claim documents.
7. Most of the original medicine bills of ASH Meditech PVT, LTD during hospitalization period
was submitted earlier but a few were left which are being enclosed herewith as enclosure.

Hope you understand my circumstances which led to shortfall in providing all required
documents in time. Therefore, it is requested to kindly grant the medical bill to aid my plight.
