GENERAL REPORT FOR MOSSES ERNEST OPIYO ACCIDENT AT

PIER 1 (NEW WAMI BRIDGE PROJECT)

At 07:15 date of 08 January 2022 Mosses fall down at pier one during conduct his daily
activity in a course of employment. After the fall he was sent to MSAMWA dispensary at
Msata for first consultation and first aid. The patient was stabilization, given pain killers,
wound cleaning and drips. After then the company management found military ambulance to
rush the patient immediately at KITENGULE hospital at Tegeta Dar Es Salaam for further
treatment at 8:45 am.
The patient was arrived at Tegeta KITENGULE hospital at 10:00 am, immediately the doctor
inspect consulted the patient and suggest the following examination and treatment;
 Arm x-ray
 Fore arm x-ray
 Femur x-ray
 Pain killers injection
Then the doctor discover the following
 Broken left leg
 Dislocation of left elbow bone
After discovering of the injuries to the patient then the patient was given referral to MWANA
NYAMALA hospital where he was also referral to MUHIMBILI national hospital. The
reason reason of going to MWANANYAMALA hospital before going to MUHIMBILI is to
change the patient title from private patient to government so as to reduce medical expenses ,
normally when a patient is going strait from private hospital to MUHIMBILI they face a huge
medical expenditures. The patient was rushed to MWANA NYAMALA and MUHIMBILI
hospital by an ambulance of KITENGULE hospital.

At 01:45 the patient was reach to MUHIMBILI hospital and immediately send to emergency
department for examination. Then the following examination was done;
 Chest x-ray
 CT scan was recommended
The patient was receive medical emergency treatment and given referral to MUHIMBILI
orthopaedic institute (MOI) for further treatment. At MOI the patient was examine with the
following test;
 Full blood picture
 Blood group and Type
 Pelvis x-ray
 Elbow x-ray
 Ankle x-ray
 Right Toe x-ray
After the examination at MOI the following was discovered
  Broken femur at the left leg
 Huge blood swell on the broken femur
 Broken right foot toe
 Left elbow dislocation
 Minor injuries on his both left and right palm
 Punctures on left side of the forehead
 Minor nose puncture
The following treatment was done
 Reallocation of the left elbow bone and supporter
 Treatment and medicine to reduce blood swell of the left femur for surgery
preparation
 Pain killers medications
 Admitted to word number 3A of MOI
The patient was recommended for surgery on his left femur. But the doctor recommend the
surgery need to be done as soon after the reduction of blood swell. The patient is doing well
and he still at the hospital receiving nurse care and medical treatment waiting for father
instructions from the doctors.

On 17/01/2022 the patient was supposed to have surgery but it wasn't done due to unknown reasons.
Then the doctor reschedules his surgery to another day.
From 18 to 19 of January 2022 the patient was still proceeding with hospital care and preparation for
surgery.
At 20.01.2022 finally, Mosses Ernest Opio was taken to surgery of his left leg femur. After surgery,
the patient was in pain and the doctors prescribe some medicines for painkillers. Also during the
process of transferring Moses from the operating bed to another bed, somehow the nurses accidentally
lead him to dislocate his right-hand elbow.
At 21.01.2022 the doctor prescribes him some additional medicines to help him recover and reduce
some pains.
At 22.01.2022 the patient's surgery wound was cleaned and inspected if there is any problem or issue
but it seems to be good. Then the doctor text to give the patient simple exercise to see if the leg can
sustain simple movements While he is sleeping on his bed. After the exercise, the doctors appreciate
the recovery of the leg and they give him one more day for medication and observation before
deciding to discharge him.
At 23.01.2022 Moses was doing well and the doctors appreciate his fast recovery then they decided
to discharge him for the next day after the morning round. Also, they keep insisting the patient keep
eating protein foods for bone fast recovery. The doctors instruct Moses to attend the clinic for his
exercises and observations after his discharge.
At 24.01.2022 the doctors were satisfied by the of the patient and they decided he can be discharged
to the Hospital. But also all the processes from the worker's compensation fund (Wcf) were completed
and the patient was given the form that will permit him to get every treatment that he'll need until his
complete recovery. Then Moses was clear to go home but the safety officer decided he is needed to
stay at the hospital until the doctors relocate his right-hand elbow.

The patient is doing well where his facial wounds and scratches are completely recovered and
his right leg toe is doing well. The company assured the patient will keep on getting his
medication until his recovery by handing over two worker compensation funds wcf they will
be responsible for every treatment needed.
