Contract for Drivers Education Collins Hill High School, Gwinnett County

The classroom portions will be held on the following dates and times: Dates:_________________________Times:________________Room:_____________ It is understood that the cost of the Drivers education course at Collins Hill Community School, part of the Gwinnett county School System is $320. This is for 30 hours of classroom instruction and 6 hours of road instruction for a total of 36 hours. Students do not receive their certificates until all 36 hours are completed, with a classroom average of at least 70. If additional road instruction is requested by the student , a fee of $40 per hour is charged. Certificate replacement---$5.00. It is understood that the student must have a learners permit before taking the class.
It is also understood that it is the responsibility of the parents to provide transportation for the student to and from the Drivers Education classes located at Collins Hill Community School, 50 Taylor Road, Suwannee, Georgia 30024. The telephone number is 770-682-4132. All Gwinnett County school rules are in effect while a student is on campus and while participating in the driving portions of the class.

I understand that the completion of the course does not in any way, directly or indirectly, imply that I will receive my drivers license from the State of Georgia or any other state. Notification of cancellation must be received within 2 business days prior to the start of the class in order to receive a full refund. No refunds are given once the class has begun. The lessons may be forfeited if the student does not attend the classroom portion or neglects to notify the driver education instructor that he/she is not able to drive during the assigned time. The student has 90 days from the last day of the classroom portion to complete the driving portion unless pre- arrangements have been made. The Community Schools liability insurance follows the car and provides the maximum coverage possible ($500,00/person/accident/property). Parents and /or guardians of students who operate a Gwinnett County Public Schools Drivers education vehicle may incur personal liability in some driver-at-fault accidents. Each instructor is certified by the DDS. Collins Hill Community School is licensed by the DDS in accordance with Title 43-13-1. A performance bond of $2,500 has and will be maintained for the protection of the contractual rights of the students. ****Note: Due to a program implemented by the DDS, your student will receive their Driver Education Certificate in the mail 5 school days after they have completed their driving requirements.

Driver Education classes are offered at Collins Hill High School during the afternoon, Monday through Friday from 2:30 p.m. until 5:30 p.m. for two weeks on the above mentioned dates. Driver Education classes meet for 30 hours of classroom instruction and 6 hours of actual driving time. The driving time is scheduled with the instructor on an individual basis. These times are arranged with the instructor for after class or on weekends. Students are scheduled two at a time in the driver education car and are picked up and dropped off in the faculty parking lot (by the Band room) at Collins Hill High School. Transportation to and from Collins Hill must be arranged by the parent. All driver education instructors are state certified, and are teachers or security officers in the Gwinnett County school system. If the student successfully meets all of the requirements of the course, a certificate is mailed to their home address 5 business days after the last driving time (duplicate certificates will be provided at a cost of $5.00 each). It is necessary to show this certificate each time you apply with an insurance company for a discount. Please check with your insurance company to determine the discount, if any, which is offered. Driver Education classes are filled on a first-come, first-serve basis. A completed registration form with student and parent signature, the students drivers license or learners permit number, and your check in the amount of $50.00 as a deposit, are necessary to register-you will be required to pay the balance of $270 by the first day of class. You may pay the total amount of the class ($320) when you register if that is your preference. Checks are made out to Collins Hill Community School. If you prefer, payment can be made with your Visa or MasterCard credit card.
