FIVE STEPS ON HOW TO ENDORSE LICENSE TO ARIZONA

FOR APPLICANTS LIVING AND EDUCATED ABROAD

1. CREATE AN ACCOUNT IN ARIZONA NURSE PORTAL in this link

1.1. Click Apply License


1.2. Apply for RN/LPN Endorsement Licensure Requirements for Foreign Educated Applicants

Applicants who have passed NCLEX but were NOT issued a license need to apply for
RN(Registered Nurse) by Exam with Foreign Education . You do not need to re-test.
Call or email your original state board of nursing for the instructions on how to process
Score Transfer.

1.3. Under Citizenship Status, Choose Qualified Alien if you are still in the Philippines and upload your
Approval/Receipt Notice and Citizenship Documentation Packet (mark the letter “M” under Alien
Status Declaration indicating that you are foreign national who is not physically present in the US).

1.4. If you are still outside US, do not request for Temporary License. This is only applicable
for nurses who are physically present in Arizona who needs immediate license to practice
while completing their endorsement requirements.
1.5. Pay the required fees. Use only US issued credit cards. Non-American issued credit cards
are not allowed.

2. UPLOAD AN AFFIDAVIT

2.1. You need to upload your scanned Passport and Affidavit Regarding Lawful Presence
Documentation/Social Security for Foreign National Applicants Living Abroad form in the message
center of your Arizona Nurse Portal account. You can download this form in this link
https://www.azbn.gov/licenses-and-certifications/forms-and-documents

3. VERIFICATION OF YOUR ORIGINAL STATE OF LICENSURE

3.1. If your license is under Compact States or under Nursys, go to this link
https://www.nursys.com/NLV/NLVTerms.aspx and endorse your license to Arizona and pay the required
fees.
3.2. If your original state of license was not part of Compact state/, complete the Verification
Form and mail (not faxed or emailed) to the state/territory of your original license. It i s

4. VALIDATION OF ENGLISH LANGUAGE SKILLS

4.1. International English Language Test Service Academic Examination (IELTS) with a
minimum score of 6.5 on the Overall Band Score with a minimum of 6.0 on each module.

OR

4.2. You can send your VISA SCREEN Certificate from CGFNS instead of IELTS. You need to login to your
CGFNS Account and make a service order “Additional CGFNS Services-Visascreen ICHP Certificate
Verification Letter” and pay $100 dollars. Make sure that Arizona Board of Nursing is the recipient.
Submitting of Visa Screen directly from the applicant is not allowed by the board.

5. FINGERPRINT CARD

5.1. Login to your Arizona Nurse Portal and request the board in the message center to send to your address, a
hard copy of a fingerprint card. You will receive a copy within 1 to 2 weeks. Or you can buy online,
example in Shopee, Amazon, etc. just look for Fingerprint card, FD-258.
5.2. Fill out the fingerprint cards and have your fingerprints taken by an NBI personnel. Check this link for

5.3. Send the accomplished fingerprint cards to Arizona Board of Nursing Address and wait for 8 weeks for the
result.

ADDITIONAL INFORMATION:

In order to accomplish the Validation Practice, an applicant should meet one of the following:

1. Practiced as a nurse for 960 hours or more in the past 5 years  or
2. Graduated from a nursing program and obtained a degree within past 5 years  or
3. Completed an Arizona Board approved refresher course in the past 5 years  or
4. Obtained an advanced nursing degree in the past 5 years (i.e. LPN to RN, RN to BSN,
masters, or doctorate)
5. Once your license is issued, the board will place your license on inactive status until such
time that you present to the board your Green card documents.
