PLANNED MAINTENANCE SERVICE AGREEMENT

We hereby agree to render the following service:

1. Check electrical system. 7. Check and lubricate unit where required.


2. Check and adjust all electrical wiring and connections. 8. Check, lubricate and adjust drive unit.
3. Check drive and lift motor brushes and operation. 9. Check and adjust brakes as needed.
4. Check and clean batteries. 10. Check all wheels for wear.
5. Check hydraulic system. 11. Check and adjust steering.
6. Check hydraulic cylinders and packings.

Work as specified above will be performed once every , for a period of at a rate
as specified above or per visit.
All work to be performed during regular working hours .
The above charge will completely cover the planned maintenance operations listed.
Units must be available for servicing upon arrival of a mechanic at location or an additional labor charge at
current rate per hour will be made for waiting time.

Parts which require replacement and can be replaced during this planned maintenance check will be furnished
at the regular parts price.
A report will be made at each of these service periods on any part of unit that requires additional work. These
repairs will be made at current rate per hour, plus any additional travel and expenses involved, and price of parts.
Special Instructions:

This agreement may be cancelled by either party on a thirty (30) day notice.
