JOINT AFFIDAVIT OF COHABITATION

(BASED ON ARTICLE 34 of the FAMILY CODE)

1. We are in a common law marriage.

2. We have been living together for 11 years already or since the year 2011. We have no legal
impediments to marry each other.
3. At the time of the commencement of our cohabitation we both continuously have no legal
impediments to marry each other up to the present;

4. We already have THREE children during our period of cohabitation, the birthdays of our child is:

5. We now swear under oath such fact of over five years cohabitation and our intent to voluntarily
and freely marry each other without the presence of fraud, force, duress, influence or
intimidation;

6. We are executing this Joint Affidavit of Cohabitation to avail of the exemption from the requisite
Marriage License as provided by our Family Code and to support our intention to marry.

IN WITNESS WHEREOF, we have hereunto signed this Joint Affidavit of Cohabitation this
