JOINT - AFFIDAVIT OF
TWO DISINTERESTED PERSONS

We, and both of legal age, Filipinos, both married and both residents of Nangka Balamban, Cebu,
after having been duly sworn in accordance with law, do hereby depose and say, THAT:

1. We personally know parents and and their (Kate Lyn, for brevity) because we are friends and
neighbors for a long time.
2. Kate Lyn was born on January 2, 2019 at Balamban District Hospital. Her birth certificate was not
registered within the reglemantary period for the reason that the parents was of the impression that the
hospital were to register the birth certificate as what happened with the parents’ first two children.
3. We are not in any way related to by affinity nor consanguinity;
