Weekend Call
● Resident on call Friday/Saturday night should update Saturday/Sunday on-call
resident of any foot & ankle cases over weekend.
● Sunday Call - Need to make the OR schedule on google drive (goodsampodiatry@gmail.com ; pw: bruce013). Add
cases to the list from the following:
1. Cases from St.Catherine’s Hospital - SCS OR LONG schedule
 Search for: Ravitz, Gamez, Boccio and any podiatrist (except Kohan for now)

2. St.Charles Hospital - SCH OR LONG schedule


- Gamez, Wagner, Javed or any other podiatrist

3. Good Sam - GSH OR LONG Combined (North & South)


 Call/text attendings the night before and ask them when they want to come in to round
 For ortho foot and ankle cases on the weekends -- discuss plan for weekend coverage of
cases between pod resident on-call & the pod resident assigned to ortho.

How do I add patient to the list on Epic?


1. Search patient’s name on top right hand corner list → right click on patient → treatment
team -> add me (click NO ED provider).
2. Good Sam → Units → right click on patient -> treatment team → add me (NO ED
provider)

Dictation
 Dial 5285 in-house; 631-376-5285 (from home / cell phone)
 Enter personal residents’ Star number
 Enter work type number (for Surgical dictation at Good Sam – 6; St.Catherine’s - 5)
 Enter patient ACCOUNT number at GSH (not MRN). Enter CSN number for St.Cats (not MRN).
 To record over a mistake, press 3 as many times as you need to playback to the spot and then press 2 to start
recording over
 -Press 9 to end dictation
 Always state, “Please send dictation to my inbox as well as fax to Podiatrist.”
 Check inbox on lower part of epic to edit dictations (24-48 hours completed after dictation)

How to take and send for a Wound Culture at Good Sam


A. Wound culture in ED
1. On Epic go to Manage Orders  PNL Wound Culture (under comment section of PNL wound culture -
1.your epic id/ name, 2. Date, 3. Where you collected specimen from (ie right foot ulcer), and 4. Beeper
number.
2. Write the same 4 things on patient label placed on test tube / culture tube
3. Place culture tube into clear bag.
4. Hit 10  Send in tube to microbiology
B. Wound Culture on the Floor –> MUST get patient’s nurse to witness the wound culture taken. Put order in the
computer / EPIC for “PNL Wound Culture”. Then in the details of the order put, “Right foot, right ulcer etc.”
Finally have nurse on the floor print, “Moby Lab Sticker,” and give her the culture. Nurse will then send to
microbiology.
a. Note: this can also be done in the ED, however it can be difficult to find nurse in ED on busy night.

Orders for Joint Aspiration (1st MPJ, Ankle joint aspirate etc.)
- You only need to get 1 Purple Tube and 1 White Sterile Cup from clean utility
- Nurse must be at bedside if aspirate is performed on the floors. Advise nurse you are performing aspirate if
performed in ED
- Put in the following Orders on EPIC before taking the aspirate:
1. Body Fluid Cell Count (Purple tube)
2. Body Fluid Crystal
3. Body Fluid Culture → WHITE CUP
4. Gram Stain Only

Bone Biopsy
1. 2 cups
a. Cup with formalin (obtain from pathology lab)
b. White cup with nothing in it for bone culture (to be sent to micro)
2. 5 patient name labels from patient’s chart
a. 3 labels go on the paper filled out to pathology
b. 2 labels to go on each of the 2 cups to pathology and microbiology
3. Orders to be placed in chart: Bone culture (white cup)

Placing a Consult
- Before placing a consult check previous chart notes (from previous admission) to see if patient has previously been seen
by different vascular / ID group / pulmonologist or cardiologist etc. than the one you plan to consult.
1. Call the operator- ask to be connected to the, “answering service,” of the doctor you need.
2. Place the consult with the secretary (provide patient’s name, DOB, location, and
diagnosis).
- Then put the consult in New Orders (inpatient consult to _”Dr.Chu,” etc.)
3. We usually just place consults to ID and vascular surgery.

New signout/ Updating signout


- NEW signout: type .signoutpod and type all information needed
- UPDATING signout - press edit, refresh button to refresh labs, change the abx/plan as needed
The person on call should update the signout the night before so it is ready to print the next
morning prior to rounding

Booking an OR case
● For surgery today: 4088
● For surgery tomorrow: 3520 (3522 before 11:00AM )
● For surgery later than tomorrow: 3286 / 3522
● Off hours/weekends/add-ons: Call the OR at 4088
● Information you’ll need on the phone call:
- The surgeon, pt name, pt location, pt birthdate,
- Planned procedure, diagnosis, and any needed equipment you’ll be needing (VAC,
vanco-powder, TPS [ power equipment ie TMA …)
 Find out when doc/podiatrist is available, what equipment you need

Pre-OP Orders
-NPO after midnight
-General Procedures to nursing- “Hold AM doses of anticoaguation on *day*”
-Clearance has been requested for: (medical, vascular, cardio)- “please provide clearance for
OR tomorrow- type of sx”
-”ANCEF NS” - for in-patients going to surgery who do not have abx or ID following  click: “on call” for prophylaxis
& select clinical indication→ surgical prophylaxis

Post-Op Ambulatory Surgery Instructions


1. Brief Op Notes
a. F2, fill everything out
2. Med Reconciliation
a. Review Orders for Discharge
i. “Mark all don’t prescribe”
ii. “Mark all resume”
iii. Resume home meds
b. Orders
i. Ask attending if patient has pain medications / antibiotics to be sent to pharmacy X-ray
ii. Always order Xrays (3 views of foot) if you placed hardware. X-ray for soft tissue cases (aside
from gas case) do not need to be performed.
iii. WB status (with surgical shoe)
iv. Ice
v. Elevate
vi. Crutches
vii. PT Eval - Reason- include post-op crutch training and put WB status of patient

c. Review and Sign


3. Follow- Up
a. Fill in attending information
4. Patient instructions - use .postopinstructions -> F2 everything.

For post-op in-patients who are going to OR→ Floor


 Click Inpatient to Floor
- Brief Op Note
- Medicine Reconciliation
- Discontinue NPO and start diet (diabetic / cardiac)
- Discontinue Generic Procedure Order placed previously to hold anti-coags
- General Procedure: “please reinforce dressing if strikethrough is noted”
- X-rays STAT (bedside port)
- PT eval
- Weight-bearing status

Discharging Instructions – this should be used when the attending tells you a patient is stable for discharge from our
stand-point on the floor. Click on the following tabs EPIC on patient’s chart:

- “Follow up” within a week
- “Patient Instructions”- here you should include weightbearing status and out-patient Wound Care Orders (i.e…
“Please apply betadine with dry sterile dressing to left foot daily..etc.” OR “Keep dressing dry and intact until follow-up
with attending.” OR “Change dressing every 2 days”).
