Dear Sir,

With due respect, My name is Mohasin S Sheikh, father of Zeeya M Sheikh, who is studying in
10th Standard in Mahanadi Section of your repudiated School. I have completed all the legal
formalities for the change of my daughter's name from “Zeeya Patel” to “Zeeya M Sheikh”. It’s
a humble request you to change my daughter's name on all the officials record of school. I also
request you to change the name in upcoming Board exam marsheet, so that it didn’t cause any
problem in future. I shall be very grateful to you in this regard.

As per Dixit Sir. guidance I have attached all the needful documents which are mandatory. The
following documents are as followed.

01). Central Gazette.

02). Aadhaar Card.
03). News Paper advertisement.
04). Marriage Certificate.
05). Notary.
