Cases seen:
Abdomen – Primary school age with thalassaemia minor. RFE: Lethargy x few months –
anaemia from thal (assess background not transfusion-dependent, rule out other causes of
lethargy). PE: Moderate splenomegaly, hepatomegaly 3cm below costal margin.
Cardiology – Primary school age with TGA, corrected PDA and intact septum. RFE: Fitness
for school sports camp (assess good functional status, no heart failure). PE: ESM LSE
radiating to back (residual PS), ESM radiating to carotids (residual AS).
Neurology – Primary-school age with tuberous sclerosis (hidden diagnosis; PMH given is
epilepsy, learning disability). RFE: Parental concern re: whitish patch on elbow; also
caregiving stress re: patient’s self-care. PE: Lesion of concern – Ash-leaf spot; other ash-leaf
macules seen on back, angiofibroma on forehead. Expected to examine rest of skin, do
adapted motor assessment for long-tract signs, auscultate heart.
Respiratory – Primary-school age with b/g asthma, ex-prem 24weeks with chronic lung
disease, Autism Spectrum Disorder. RFE: Collect asthma medication – poorly-controlled
asthma non-compliant secondary to caregiver steroid-phobia + no supervision; also has
some growth restriction. PE: Normal respiratory exam apart from subtle Harrison’s sulcus;
height restriction + baseline low weight.
