IV. Health Declaration

I, the undersigned Official Veterinarian, hereby certify that:

1. Ireland is free of Foot and Mouth Disease (FMD), Anthrax, African Swine Fever, Classical Swine
Fever (hog cholera) and swine vesicular disease for the past 12 months prior to and until the date
of export.

2. The pigs (from which the products are derived) have been continuously resident from birth, in
registered farms under the continuous supervision of the veterinary authorities of Ireland.

3. The farm(s) (from which the products were derived) have been certified to be free from swine
vesicular disease, and no case of former list A OIE infectious or contagious pig diseases have been
diagnosed or occurred at the farm in the preceding (6) six months prior to export.

4. The pigs (from which the pork meat or pork products have been derived) have been slaughtered
and processed in an abattoir or processing plant which has been approved for export by the DVS,
Malaysia and has been under continuous supervision of the veterinary authority of Ireland.

5. The pork meat or pork products were derived from pigs that have been subjected to ante-
mortem and post-mortem inspection and were found to be healthy and free from any clinical sign
of infectious or contagious disease at the time of slaughtering/processing.

6. The pork meat or pork products have been processed, packed and stored under sanitary
conditions and do not contain any preservatives, colouring matter, residue or any foreign
substance or harmful material injurious to health and every precaution has been taken to prevent
contamination during processing, packing, storing and handling prior to export.

7. The pork meat or pork products destined for export to Malaysia are safe, wholesome and fit for
human consumption.
