AFFIDAVIT OF CONSENT TO TRAVEL ABROAD

I, GERALDINE P. AGUM, of legal age, Filipino, single, and a resident of

Purok 12, Baloy, Tablon, Cagayan de Oro City, after having been duly sworn to
in accordance with law do hereby depose and say:

1. That I am the mother of Shaun Wayne Gabriel Agum who was born on
November 8, 2004 and Nasir Agum who was born on August 12, 2007;

2. That I hereby give consent to have my children be traveling abroad at

Dubai, UAE together with my parents namely Glenn A. Agum under passport
number TT0832827 and Rhoda P. Agum with passport number W0625239;

3. That my parent will be traveling to Dubai UAE, on the month of

December 2007;

4. That I will be the one to support for the travel of my children in going to
Dubai, UAE as at that time I will be already in Dubai, UAE.

5. That this affidavit of consent is executed to attest to the truth of the

foregoing facts.
