hereby depose and say:

1. That I am the biological mother of Stephanie Nichole Y. Angeles, who

was born on September 11, 2003 at Cebu City;

2. That I am giving my free and voluntary parental consent to my

daughter, Stephanie Nichole Y. Angeles to travel to Hongkong for a tour, I also
authorized and to give consent to Susan Angeles Arreza to be the traveling
companion of my daughter;

3. That this affidavit of consent is executed to attest to the truth of the

foregoing statements and for whatever legal purposes it may serve.
