AFFIDAVIT OF LOSS

I, ELTON EDWARDS, of legal age, married, Filipino, and a resident

of Sta. Crus -1, Lapasan, Cagayan de Oro City, after having been duly
sworn to an oath in accordance with law, hereby depose and say:

That I am the owner of a brown wallet which contains the following

articles: my Driver’s License, 2x2 ID photos, and my Stonerich Company
ID;

That said wallet with its contents was inadvertently lost from my
possession while I was driving my motorcycle and traversing JR Borja
Extension to Gusa, Cagayan de Oro City, on July 7, 2019, at about 8:51 in
the evening;

That despite efforts to locate the said wallet, the same proved to be
futile;

That I am executing this Affidavit to attest to the truth of the

foregoing and for whatever purpose this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 8th day of
