    compose the team of Pagadian-Zambo. Sur-Sibugay regional Football Association (PaZSS RFA) for

    Philippine Football Federation U19 National Championships at Dumalinao Regional Pilot School,

    Dumalinao, Zamboanga del Sur on March 26, 2022.

    I agree to assume all risks due to injury that may occur as a result of my child’s involvement

    and/or travel to and from said activity.

    I understand that by allowing my son, I agree to release PaZSS RFA, its coaches and staff,

    from and against any and all liability, loss, damages, claims or actions to the maximum extent

    permissible by law, arising out of such participation.
