We’d like to ask you some questions about your __-year-old’s experiences over his/her lifetime and
the past 12 months.
As I mentioned earlier, we are conducting a survey for the University of New Hampshire on the
experiences of children in the United States. This study is being supported by the U.S. Office of
Juvenile Justice. For this study, we will be interviewing 4,000 children and parents across the
country to find out about potentially stressful circumstances that children sometimes confront, and
how we may better protect kids from dangerous situations. This is an important study that will give
us a better understanding of the kinds of problems that children face and help us to better plan for
the future needs of children and their families. We would like to interview you concerning your
child [child’s name]. You have been randomly selected to represent parents of children in [child’s

This interview will take approximately 30-40 minutes and can be done at any time that is
convenient to you. We will be asking you about things that may have happened to [child’s name]
at school, in your neighborhood, or at home. We will also ask you some questions about his/her
health and behavior. This interview is completely confidential; you or your child’s name will not be
recorded or linked to the answers that you provide. The interview is completely voluntary – you
can, of course, decline to participate in the interview or can refuse to answer any particular
question. At the end of the interview we will send you a check for $20 as a token of our

with that of over 4,000 other children to help us better understand the kinds of risks that children

We would like to ask you to participate in an important study. This is part of a national survey of
over 4,000 children and teenagers. We are interested in your experiences, so that we can better
understand the kinds of things that young people go through. In this interview we will be asking you
some questions about stuff that may have happened in your school, in your neighborhood, or at
home. We’ll also be asking you about your health and how you’ve been feeling lately.
You were chosen completely at random to represent the experiences of thousands and thousands
of young people your age. We do this by dialing random phone numbers until we find someone
the right age. You don't have to participate in this interview if you don't want to, but your help will
make a big difference.
The interview will take about 30-40 minutes to complete. We will be asking you some questions
about dangerous situations you may have faced and about some personal situations, where you
might have been threatened. You can stop the interview at any time. If this is a bad time to talk, I

could call back at a better time for you. To thank you for your help, we will be sending you check
for $20.
Everything you say will be completely confidential. We are not going to tell your parents, your
school, or anyone else anything you told us. We are going to be combining your answers with
those from thousands of other kids to present a picture of the experiences of young people in the
United States.
