METHODOLOGY statement. Material required for every quality

1. Outpatient come to the rehabilitation department with appointment or walk in.
2. Patient appointment come to department with appointment card/appointment
time.
3. Patient walk in either verbal order/booking via phone.
4. Time of register and time of treatment has taken and documented. (Outpatient
log book)
5. All type of treatment documented. (Patient folder)

CONCLUSION
For period of 1 April - 30 April 2020. All patient get treatment <7 minutes which is

100 % achieved target and 0 patient get treatment >7 minutes.

Give treatment to the patient as soon as possible and to continue monitoring.
