SCHOOLS DIVISION OF QUEZON PROVINCE
I. PURPOSE
Poctol Elementary School is a non-central public elementary school in the District of
Pitogo located at Brgy. Poctol Pitogo, Quezon, 10 minutes away from town proper. Teachers
are collaborators in learning, seeking new knowledge and constantly acquiring new skills
alongside their students. And also set a vision for creating learning experiences that provide
the right tools and supports for all learners to thrive.
Due to this increasing demand for a safe, comfortable and convenient facilities to
promote a conducive place for learning the caters the variety of learning profile Poctol ES
take an initiative to develop a school base project known as Project ASAD (Alertness and
Safety Against Disasters).

E. Provide support the maintenance of the school facilities including school basic
utility service such as water supply, electricity and telecommunications.
F. Provide the installation of other school materials that strengthen the safety of the
stakeholder.
III. SCOPE
The coverage of this plan is for the whole year. It involves the school head, school
property custodian and SDRRM Coordinator. It includes the procurement of necessary
equipments for safety of our learners and school personnel and to maintain the cleanliness
and orderliness of the school to prevent untoward incidents and spread of different
diseases. The following are the plans for the whole year:

The monitoring and evaluation activities will conduct by school level/district level for
Pre-Monitoring activities to collect the necessary data for actual monitoring. The actual
Monitoring Activities will conduct by district/division level for confirmation and validation
of data and providing technical assistance. The Post-Monitoring activities will be conducted
by school level to comply to the necessary data. The data collection and management will be
conducted thru onsite modality.
Data gathering procedure will use the checklist and survey questionnaire. Those
tools will help to determine the development and to assess the areas to improve in this
project.
The data collected will be organized according to its areas of concern such as
Preparation and Mitigation, Preparedness, Response and Rehabilitation. This will be
compiled to DRRM Coordinator’s portfolio for future references.

V. TERMS OF REFERENCE
The members composed of School Head as the Chairperson and SDRRM Coordinator
as Team Leader. The members are composed of School Property Custodian and School
Health Coordinator. School Property Custodian helps to monitor and conduct inventory on
ncessary equipments and supplies related to DRRM. School Health Coordinator helps to
monitor the health and safety of learners, school personnel and guests within the school
premises and provide medical support for all activities conducted within the school. The
SDRRM Coordinator serves as a focal point for collaborations and coordinations related to
safety and orderliness in the school.

The process of analyzing gathered data is through the use of tabular form specifically this
will adopt the existing M&E tools such as checklist, survey questionnaire and interview.
The checklist contains the areas of concern for each pillar of DRRM and the necessary
supplies, equipments and reports for each pillar or readiness checklist. Survey
questionnaire may contain the readiness questions for each areas of concern. The interview
may conduct by monitoring team to assess and evaluate the data for improvement.

The estimated amount for funding is amounting to Php 10,000.00 from MOOE.
(Final Note: The SGOD-SMME provides this to aid schools in the crafting of their School
M&E Plan as per DO 029, s. 2022. You may localize this M&E Plan based on your needs.
You can place it in your school’s stationery template as per the DepEd Manual of Style
[DMOS] and the DepEd Service Marks and Visual Identity Manual [DSMVIM]. This Plan
may be adjusted based on results of progress monitoring. The year-end results will serve as
input in developing or improving this Plan in subsequent school years of the SIP—year 2
and year 3.)
