    Good day!

    May I respectfully request from your good office to reconsider the payment made on
    March 11, 2020 with regards to the remittance of the employees of SALES LAW OFFICE
    (Employer ID No. 800174114969). That in the HQP-PFF-053 form (Employer Contributions
    Payment) the applicable period was indicated thereat at February 2020 instead of March 2020
    and May 2020 instead of June 2020 which is the true and correct applicable period. That the said
    mistake was committed because the clerk who personally paid for inadvertently wrote February
    2020 instead of March 2020 and May 2020 instead of June 2020.

    Hoping for your favorable action on this matter.
