through the follow: Taper/Ramp: for medication such as

combine 2 meds together (carbamazepine 200
+400) or different dosing of aspart (5 units in
the morning and 7 units before lunch) or

The maximum dispense quantity per KFMC policy is

3 months, the remaining will be provided in future
refill

Make sure you choose the right pharmacy before signing the order (patients located in the Main
Hospital AND Pediatric Hospital will receive the medication from Main Hospital Discharge Pharmacy –
patient located in Rehab Hospital and Woman Specialty Hospital will be dispensed from Woman Special
Hospital Pharmacy). During the weekend ALL discharge medication should be entered in the Main
Hospital Discharge Pharmacy ONLY
