Mayor Contractor Trust Agreement

___________________________________________,do solemnly pledge to

Mrs. Imelda Zamora, Director for Upland Linkages, CAR and
___________________________________________________in regard for
(Name of Contractor)
the upcoming project/ projects which shall be funded by the National
Government and forwarded through the LGU in our municipality.

Henceforth, in the event that these financial resources would arrive

and agree to release in full this said funds to the chosen contractor, minus
the SOP, this budget therefore, shall be intended for the implementation of
the project/ projects listed below.

In view thereof and in exchange for this commitment, the above-

mentioned name of the contractor shall conceal to the following terms and
conditions:

1.)In the event that the appropriated Municipal project /projects would
commence and start. The contractor will make sure that he/she
must always be there at the project site. If ever he/she has to leave
a documented written statement with a valid reason must be
stated, which then shall be signed and approved by the Municipal
Mayor.
2.)The contractor shall enter into an undertaking in regard to a hold
departure order from leaving the country with the authorized
government agency/ person. This shall only be lifter if the funded
project/projects have been fully accomplished.
3.)The contractor shall allow inspection of the project/projects from
time to time, together with its’ legitimate summary expenses.
4.)The contractor must never use the allotted funds for any other
purposes.

Failure to comply to the above terms and conditions by the assigned

contractor, will therefore be strictly punishable by law.

This Mayor, Contractor Trust Agreement shall therefore be binding and

legal until it is expressly revoked by the above-mentioned authorized
facilitator in view of the execution for the declared foregoing contract.
 In addition, any cancellation done by the contractor in relevance to the
awarded project/projects, shall be penalized forty percent (40%) from every
project cost, which shall be paid by him, and simultaneously be banned from
taking or entering any other projects facilitated by Mrs. Imelda Zamora with
the different Municipalities of Benguet.
