    The undersigned are presently conducting a research study entitled “The Impact of COVID-19
    in the Reading Readiness of the Grade 10 Student of Tambisan National High School SY-
    2022-2023” as a subject requirement in the Practical Research II subject.

    In line to this, the undersigned would like to ask permission to float survey questionnaire to
    Grade 11 and Grade 12 students in the school. Rest assured that whatever data collected from
    this survey will be used for research purpose only.

    Attached is the copy of the survey questionnaire for further reference. We are fervently hoping
