    Greetings!

    I would like to ask for your permission to conduct home visitation to the students listed
    below:

    1. Mucas, Leo Jr. A.


    2. Mambot, Jennifer M.

    The students listed above have not attended our school for a couple of days now and I
    would like to pay them visits to check them and to talk with their parents/guardians about
    their status in school.

    If pemitted, I would like to pay them visits during my vacant time, 8:30 AM to 10:45 AM or
    after class.
