    Greetings of peace. I am JOCELYN B. BERBA, a residence of Zone-1 P.C. Hill, Rosary Heights
    1, Cotabato City and recently passed the Licensure Examination for Teachers held on October
    2, 2022 at Koronadal City.

    During the application for the October 2022 LET, I had typographical error in inputting my
    birthdate. The date registered on my Leris account was ____________ while the correct date of
    my birth was _____________. Due to this mismatch, I cannot process my application for oath
    taking.

    With this, I would like to request for CORRECTION OF BIRTHDATE for my LERIS Account.

    Hoping for your immediate action to this request as I am planning to take my oath as a licensed
    professional teacher on ____________.
