Subject: In reference to one of the requirements for EPS II position

Dear Madam:

I, HAJJ P. BORNALES, am writing this letter to consider accepting my authenticated PRC ID

License, although it has reached its expiration date. With regards to my appeal, herein are the
following reasons why I was not able to renew my license:
1. My license expired last November 21, 2019, and I got no chance to renew it because I
have a full time teaching load. I did not want to compromise my classes and leave my
students with no substitute teacher. Unluckily, there was none, so I decided not to renew
it immediately.
2. I decided to renew my license on vacation so I can’t disturb any of my classes. I
scheduled it last April 2020 because some dates such as December where I usually have
time was already full on schedule and therefore not available.
3. Due to the pandemic, I could not renew my license last April 2020 because all borders
were closed. I tried to reschedule it again, and the only available schedule for renewing a
licenses is this upcoming October.

I am humbly appealing to reconsider my request to accept my document regarding my PRC ID

License. I promise to provide you a copy of my authenticated renewed license once I have it.
However, I will respect your decision with regards to this matter.

I am hoping for your consideration. Thank you so much, and God bless.

Respectfully yours,

HAJJ P. BORNALES
Applicant
