This is in reference to your alleged violation of the Company House Rules, the factual
antecedents of the alleged violation are as follows:

“Based on the incident report, last 14 December 2022, you arrived at the 7/11
Kitchen compound located at Ilo-ilo under the influence of alcohol. Because of
the incident, a kitchen crew and the subject rider was involved in heated
discussion in the group chat. The assistant operations manager pacified the rider,
but the latter refused and continuously uttered indecent remarks in the group
chat.”

In this connection, uttering indecent remarks towards colleagues at work may constitute serious
misconduct which is a ground for disciplinary action, under the Company House Rules and the
Labor Code.

In view of the foregoing, you are hereby given five (5) days from receipt hereof, within which to
submit your written explanation stating why no disciplinary action should be taken against you
regarding the above-quoted incident.

Failure on your part to submit your written explanation within the period provided shall be
construed as a waiver of your right to be heard and submit evidence and the Company shall
decide the matter on the basis of all records and evidence available to it.
