7. Portfolio & Outstanding Accomplishment;
8. Certified Photocopy of Voter's ID and/or any proof of residency as deemed acceptable
by the school screening committee;
9. NBI Clearance
10. Omnibus certification of Authenticity and veracity of all documents submitted signed
by the applicant.

That the above mentioned documents are the faithful reproduction of the original and I
hereby attached the above mentioned documents for Ranking purposes;

That I am executing this affidavit to attest to the veracity of the foregoing facts for all
legal purposes, for which this may serve.
