Madam:
This has reference to the change of office address of the undersigned. The
undersigned is a registered taxpayer with registered office address at Unit 7,
2nd/Floor SDC II Buidling, National Highway, Brgy. 77, Marasbaras, Tacloban
City. Subsequently, an application for change of address or update of certificate of
registration was filed by the undersigned by reason of her transfer of office
address.
In this regard, the undersigned would like to respectfully request that she be
allowed by this good office to continue to use and utilize her remaining official
receipts bearing the previously registered address at Tacloban City. Nonetheless,
the undersigned has already filed an application to update her certificate of
registration. For this purpose, the following are the unused official receipts of the
undersigned: Booklet No. 1, Serial No. 000036-000050 and Booklets No. 2-10,
Serial No. 000051-000500.
