constitute and appoint my mother, MA. TERESITA J. PEREZ, to be my true and lawful
attorney-in-fact, for me and my name, place, and stead, and for my own use and
benefits, to do and perform all or any of the following and things:

1. To apply, follow up, pay the fees required and to receive the same;
2. To Sign and Execute the necessary documents, agreements, and other
