Procedure 4 3 2 1 Remarks
1. Checks the physician’s order.
2. Assembles all the equipment
Catheterization set with:
 Foley Catheter F14
 Sterile gloves
 Sterile cotton balls with antiseptic solution
 Water soluble lubricant
 Syringe with sterile water
 Specimen container
 Disposable urine bag
 Waterproof pad
 Micropore tape
3. Identifies the client and explain the procedure and reason for doing
it
4. Does handwashing
5. Provides privacy
6. Places the waterproof pad under the buttocks by asking the client to
turn to the side. Slide the waterproof pad in place
7. Facing patient stands on the left side of the bed if right handed.
Places the client in a dorsal recumbent position with knees flexed.
8. Spreads the blanket diagonally over the bed leaving the genital area
exposed.
9. Places the tray on the bed between the legs of the client. Positions
the light to give the clearest view of the genitals.
10. Puts on sterile gloves. Places a sterile towel under the client’s
buttocks while keeping the gloved hands protected
11. Opens the package catheter. Inflates the balloon with the prefilled
syringe to check if defective. Aspirates fluid back and into the
syringe.
12. With the thumb and forefinger of the dominant hand, separates the
labia and pulls the hand slightly upward to make the urinary meatus
visible.
13. Keeping the dominant hand in place, picks up a sterile cotton balls
with his/her non-dominant hand to cleanse the area.
14. Cleanses from above the urinary meatus downward with one stroke
and drops the cotton balls in a receptacle for soiled cotton balls.
15. Cleanses the labial folds from above downwards to the anus. Makes
sure that the cotton balls are discarded after each use.
16. Picks up the catheter and lubricates about 1-2 inches from the tip.
17. Gently inserts the tip of the catheter about 2 inches very gently and
slowly or until urine starts to flow. If there is resistance during
insertion, instructs the client to do deep breathing and rotates the
catheter gently until the urine starts to flow.
18. Inflates the balloon by introducing sterile water to the injection port
of the catheter.
19. Pulls the catheter gently after to feel the resistance.
20. Obtains urine specimen in a sterile container if ordered.
21. Connects the catheter to the drainage tube and checks the flow of
urine.
22. Secures the catheter with a tape on the upper thigh.
23. Dries the area. Places the specimen where it is safe from spilling.
Removes the tray and equipment from bed.
24. Places the client in a comfortable position.
25. Measures urine output. Notes the color, character and consistency
of urine
26. Sends urine specimen to the laboratory with proper label.
27. Cleans used articles and discards disposable articles according to
hospital policy
28. Documents the procedure (FDAR).
29. Maintains body mechanics throughout the performance of the
procedure.
30. Shows calmness while performing the procedure.
