AFFIDAVIT OF CONSENT and SUPPORT TO TRAVEL

WE, A and B, American Citizens, of legal ages, and residents of Amway, USA, after having been
duly sworn to according to law, hereby depose and state that:

1. We are the legitimate parents of C, a minor who is 16 years of age, having been born on
January 1, 2011;

2. Our son has been invited by his school to be part of the group that will be travelling to the
United Kingdom this January, 2011 to attend the Leadership Encounter;

3. Since our son is still a minor, we hereby freely and voluntarily give our consent for him to
travel to UK, specifically to attend the Leadership Encounter at International School;

4. In said travel abroad, my wife and I shall be responsible for our minor son’s round trip ticket
fare and for his expenses while he is abroad. We further guarantee that he will not be a public charge and
will comply with all the laws, rules and regulations imposed upon transient visitors in the country he is
visiting.

5. We are executing this affidavit to attest to the truth of the foregoing circumstances, and for the
purpose of giving parental consent to our minor son’s trip abroad and for whatever legal purpose this may
serve.
