METHODOLOGY: The questionnaire must be answered from top to bottom. Every question has to be answered.
The first question that is answered “Yes” identifies the lift category indicated in the right columns.

Step by step plan of lifting operation: (if required use reverses side of this sheet for additional information/sketches etc.)
1. Carry out TBT explain to all personnel involved the lifting operation about to take place.
2. Visual inspection of crane/boom truck and lifting gears.
3. Install berries and warning signs for unauthorized access to the lifting radius.
4. Set up crane in correct position with full extend outriggers and outriggers pad.
5. Swing and raise the crane/boom truck for hoist position above the centre of load.
6. Attach slings to the load according to the slinging method and attach tag line.
7. Once secure, take the tension on the lifting arrangement and stop.
8. Banks man shall check that the crane/boom truck hoist is at centre off the load and boom angle according to the load chart
9. Banks man shall signal to crane/ boom truck operator for lift load and manoeuvre boom to lift down point
10. Steady the load and finally adjust the load to a final lower position
11. Lift down the load and remove slings from the load and crane hoist
12. Demobilize the crane/boom truck, remove berries and warring signs.
13. Complete work with good housekeeping practices
