7. Lift Categorization Questionnaire

Routine Lift
1. Do you know weight and COG of the load? YES
2. Is the weight of the load less than 10 tonnes? YES
3. Is crane capacity utilization within 75%? YES
4. Are you using a standard rigging arrangement? NO
5. Are the lift points above or in-line with COG and are verified as fit-for purpose? YES
6. Does crane operator see the load at all times? YES
If answer to ALL questions above is YES sign off and proceed. Otherwise move to the next section
Non-Routine Simple Lift
7. Is the weight of the load between 10 and 30 tonnes? NO
8. Is crane capacity utilization between 75% and 85%? NO
9. Is COG location not in the centre of the load? NO
10. Is it a sensitive or fragile load? NO
11. Does rigging arrangement require basic slinging? YES
12. Do lift points require verification? NO
13. Does the lift require suspended load transfer (cross hauling)? NO
14. Is the work area restricted in any way? NO
If answer to ANY of the question is YES request AP approval. Otherwise move to the next section
Non-Routine Complex Lift
15. Is the load weight over 30 tonnes? NO
16. Does the lift involve use of aerial bundle extractor? NO
17. Is it a multiple crane or top and tail lift? NO
18. Does the lift require special ground preparations? NO
19. Is the crane maximum capacity above 200 tonnes? NO
20. Does crane set up require more than one shift? NO
21. Does the lift duration extend over one shift? NO
22. Is it a lift in or out of confined space or through a hatch? NO
23. Is it a blind lift? NO
24. Does the load path go over live process lines or equipment? NO
If answer to ANY of the questions above is YES request AP and SHELL SME approval.
Special Consideration
1. Does the lift involve lifting of personnel? NO
If the answer to the above question is YES seek Shell VP Production approval

REMOVAL AND REINSTALLATION OF ANGLE (MUD DRUM) VALVE
USING LOAD BEARING SCAFFOLDS AT 2E-1112A GTL T-2 BC AREA

REMOVAL OF EQUIPMENT:
. Verify the rated capacity, Validity of the load bearing & have a visual check before use.

. Secure the load with lifting tackles as per attached Rigging Tackle Arrangement (Refer the LPRA.)

. Use a proper tag line to control the load.

. Ensure the load is completely free prior to lift from the position & check no loose
objects are with load.

. Drop the load on to the pallet or timber using chain block. (Secure the load).

. Detach all the lifting tackles from the load.

. Demobilize the tools & tackles from the work location.

REINSTALLATION OF EQUIPMENT:

. Mobilize the equipment/load at the work location.

. Verify the rated capacity, Validity of the load bearing & have a visual check before use.

. After mechanical teams’ necessary preparations done, attach the lifting tackles with the
load & load bearing scaffold as per the Rigging Tackle Arrangements.

. Lift and position the load using chain block, wait for the mechanical team to align & secure
the load in place.

. After installation/box up of the equipment Detach all the lifting tackles from the load.
. Demobilize the tools & tackles from the work location.
