JOINT – AFFIDAVIT OF
TWO DISINTERESTED PERSONS

We, MERLITA A. PERRATER and CLARISA B. LIMBAGA, both of legal age,

Filipinos, both married and both residents of Poblacion II, Malabuyoc, Cebu, after having
been duly sworn in accordance with law, do hereby depose and say, T H A T:

1. We personally know NICHOLE JOHN PERRATER GENTERONE;

2. We reside in the same locality and since Nichole John’s childhood, he uses Perrater
as his middle name;

3. We likewise personally know that SUSAN LIMBAGA PERRATER, her maiden

name, is the mother of Nichole John P. Genterone;

4. We are not in any way related to Nichole John P. Genterone within the 4th degree
of affinity nor consanguinity;

5. That finally, we are executing this affidavit for the purpose of informing the
authorities especially the Local Civil Registrar and the Philippine Statistics Authority
of the above-mentioned and to support the Petition for Correction of Entries of birth
certificate of Nichole John P. Genterone who is the child of Susan P. Genterone
and Lorenzo Eroja. This affidavit is based on personal knowledge.
