City, after having been duly sworn to in accordance with law, do hereby depose
and state that:

1. I am the new Human Resource Director of PPC Asia Corporation;

2. I am aware of a labor case filed by Gerline J. Uson against PPC Asia
Corporation before the National Labor Relations Commission;
3. Upon perusal of records on file, I found no documents necessary for the
release of Ms. Uson’s final pay and other benefits;
4. There is no proper turn-over of accountabilities made by Gerline Uson to
release clearance in her favor;
5. That I am executing this Affidavit to attest to the truth of the foregoing
statement for all legal intents and purposes it may further serve.
