SCHOOL ACTIVITY CONSENT FORM

Dear PARENTS & GUARDIANS!

Greetings!
St. Jude College Dasmariñas Cavite, Inc. (SJCDC) - School of Nursing will be holding its
Capping, Pinning and Candle Lighting Ceremony (CPCLC) 2022 with the theme, “Thaddean
Student Nurses: Altruistic Advocates and Ambassadors of Compassionate &
Responsive Healthcare” on November 08, 2022 (Tuesday) at 01:00 p.m. in the Afternoon, at
Ugnayang Lasalle, Dasmariñas City, Cavite.

We know that you’re very excited to celebrate with your children and join them in this significant
milestone. In connection to this, we would like to seek your consent in allowing your child in
attending both the actual EVENT (11/8/2022) and the planned REHEARSALS (11/5/2022), as
part of the preparation. We assure that the SJCDC management will follow health and safety
protocols.
